
Your Agapé Hospice & Palliative Care team is comprised of licensed medical professionals that are dedicated to educating families and keeping our patients as comfortable as possible. Patient visits can be scheduled at any time and are typically scheduled based on the patient’s symptoms or care needs.
Our team includes
PHYSICIANS
The patient’s primary care physician works with our hospice team and physician, keeping them apprised of the patient’s condition. Our hospice physicians are highly trained and experienced in managing pain and symptoms, which will overall maximize the patient’s quality of life.
NURSES
Our patients are assigned a Registered Nurse who regularly assesses the patient’s medical needs. The nurse oversees the patient’s care plan and makes regular visits to the home or facility. Our Nurses provide hands-on care and report any changes in the patient’s medical condition. Nurses are available 24 hours a day.
HOSPICE AIDES
A hospice aide is available to assist in meeting the patient’s personal care needs. Aides help with bathing, grooming, and tidying up your immediate living area.
SOCIAL WORKERS
Social Workers assist patient’s and loved ones with grief counseling and end-of-life planning. They also assist with financial issues and accessing other community resources.
SPIRITUAL COUNSELORS
Our Spiritual Counselors are drawn from many faith traditions and are specially trained for ecumenical hospice chaplaincy.
VOLUNTEERS
Our volunteers are important members of the hospice care team. The can provide friendly phone calls as well as visits. Their visits can allow your primary caregiver to take a brief break from care and run errands. Our volunteers have been carefully screened and trained for hospice.
BEREAVEMENT COUNSELORS
Our counselors provide counseling for patients and loved ones. Agape Hospice & Palliative Care bereavement services are available for any community member experiencing grief from the death of a loved one.
